Re: Math & Science Workshops Participants – Title II

Enclosed is the list of the Title II In-Service Workshops for the 2001-2002 school year and the participants. Due to the low number of registrants for some of the workshops, we will be able to send all those who registered to all of the workshops. Please notify the staff member(s) in your building that have been approved to attend the workshops.

The Regional Office of Education will bill the district for the total number of participants registered for each workshop. Therefore, it is important to notify my office as soon as possible if the person registered, or an alternate, is unable to attend a workshop as scheduled.

Prior to attending the workshop each teacher is responsible for submitting a Form DI07, ABSENCE FROM DUTY REQUEST FORM. Please initial the form to indicate your approval and knowledge of the teacher’s day of absence. Use budget code 1-5-080-066-2210-1200-8. Then send the completed Form DI07 to my office for final approval.
